Absent Notice Form

Hi CCAA Team!

Please fill out the following form when you are not able to teach a lesson due to a conflict or vacation. This will help us reschedule lessons and notify students of any scheduling changes.

Please be sure to fill out a separate absent notice form. If you will be missing more than three teaching days, do not include the additional missed days in the comment section.

Please submit this form 3 weeks in advance of missed dates for our consideration.

Once we receive your Absent Notice Form, we will email you a confirmation email letting you know if we have a room available for the time and date you have provided.
